Granulocyte colony stimulating factor are used to improve the receptivity of the inner lining of the uterus and help in successful implantation. Its more commonly used in patients with chronic thin lining, recurrent implantation failure.
GCSF is usually inserted intra uterine or given sub cutaneously before the embryo transfer and has proven role in treating implantation failures due to thin endometrium (endometrial thickness< 7 mm on ultrasound). Normally we give one or two intrauterine applications of G CSF. if the endometrium still is refractory, we may apply intrauterine platelet rich plasma or PRP. In this 30 ml of blood is withdrawn from patients. This blood is processed using a FDA approved device. From this about 3-4 ml of PRP is extracted. 0.5 ml of prp is instilled into the uterine cavity. Our unit in lilavati hospital Mumbai, was one of the first units in India to achieve pregnancy with intrauterine G CSF many years back. Since then all our IVF units have achieved pregnancies in hundreds of patients with thin endometrium using G CSF, or PRP or a combination of the two.
